Safety Management System Review
For this assignment, you will conduct a review of the safety management system (SMS) at your organization or an organization with which you are familiar.
Some students may not currently work in the safety field and do not have access to the required information outlined in the instructions. If this is the case, do not hesitate to contact your professor to discuss an alternative method.
Many elements make up an effective SMS, as outlined throughout this course. For simplicity, you will review and discuss the following six SMS core elements for this project.
Management Commitment and Leadership
Employee Participation or Involvement
Roles, Responsibilities, and Accountabilities
Hazard Recognition and Control
Education and Training
Performance and Measurement
While the review template provided below is based on the core elements established in OSHA’s Recommended Practices for Safety and Health Programs (referenced in this unit’s Required Unit Resources), you may utilize any of the following to guide you through your review:
ANSI/ASSP Z10.0-2019 and ANSI/ASSP/ISO 45001-2018, as discussed in the textbook;
the Occupational Safety and Health Administration (OSHA) VPP Site-Based Participation Site Worksheet, contained in Chapter 18, Addendum A, of the course eTextbook; or
OSHA’s Recommended Practices for Safety and Health Programs (referenced in this unit’s Required Unit Resources).
As part of your review, you will complete the Unit VII SMS Review Template . Follow the instructions below and those listed in the template to complete this project. You will complete each step below for each of the six core elements listed in the template.
Step 1: Collect Evidence. You do not need to provide copies or images of the actual documents or reports that you review. However, you must identify all resources and specific information you examined in the first section of the template. Suggested sources of information are as follows:
Review applicable documents (e.g., safety manuals and instructions, safety policies, safe operating procedures, job hazard analyses).
Review pertinent reports/records (e.g., emails or letters from leadership to employees, safety meeting minutes, injury logs, audit reports, OSHA citations, inspection reports, risk assessments, training records).
Conduct interviews. These may include one-on-one or group meetings with managers and supervisors as well as one-on-one or group meetings with employees.
Observe the workplace for safety communications (e.g., policy postings, team or department safety meetings, safety committee meetings).
Observe the workplace for safety compliance or at-risk behaviors.
Step 2: Evaluate the Evidence. For each SMS element, compare the evidence reviewed against the stated criteria in the guidance document you are using by discussing what you found or were unable to find.
Step 3: Describe the (2-3) pros and possible (1-2) cons of your proposed innovation? In the next section of the template, you will discuss your assessment of each core element function based on your observations and objective evidence as they compare to standards, available resources, and best management practices. Identify any significant gaps. Your discussion in this section of the template must be at least 100 words for each element.
Step 4: Assign a Performance Level Rating. Use the drop-down menu in the template to rate each core element. Base your rating for each element on the following descriiptions:
Effective: There is evidence that the core element function is effective and is achieving the desired outcome. Evidence that the information reviewed is visible and documented with action planning.
Strong: There are minor gaps in the function of the core element with some action planning.
Moderate: Evidence of the core element function is scattered and is non-conformant with established criteria. There are some positive trends.
Inadequate: There is little evidence observed of the core element function. Multiple or significant systemic non-conformities exist. There is no action planning.
Step 5: Explore Opportunities for Improvement (OFI). For each SMS element, discuss the opportunities for improvement that you found based on your review.
Identify at least one OFI for each core element. Keep in mind an OFI many not necessarily be a deficiency but an area where greater effectiveness or efficiency can be obtained. The OFIs you identify may or may not expound on gaps discovered in Step 3.
Justify the OFIs based on your synthesis of course concepts.
Do not propose solutions for the gaps found or the opportunities for improvement you have concluded. This will be accomplished in the next unit’s assignment. You are encouraged to review those assignment instructions to ensure you are building a solid foundation in this unit to complete that assignment.
At the end of the template, you will summarize the overall status of the organization’s SMS, offering your opinion on the degree of alignment between the SMS and established standards or other management system efforts utilized at the facility. Your summary must be at least 100 words.
For this project, you must use at least four scholarly, peer-reviewed, or industry-standard resources, one of which may be your course eTextbook, to support your review, discussion, and/or summary. The organization-specific sources and evidence you collect do not count toward this requirement.
You are encouraged to offer your opinion and experience. Although your opinion or belief may be based on experience and not necessarily a truth claim, you should incorporate resources that support your opinions.
All sources used must have citations and references properly formatted in APA style. References will follow your summary in the template. APA formatting of the assignment is otherwise not required.

Acme General Contractors is the general contractor for a new 20-story commercial building in downtown Centerville, USA. The building will use a rigid-frame structure and reinforced concrete construction. A major subcontractor on the project who will be performing the roofing and interior insulation and drywall installation has hired you as the safety officer.
Section II: Assignment

Job Hazard Analysis
In this assignment, you will demonstrate how to conduct a hazard risk assessment by applying unit concepts to the simple task of changing a tire. Keep in mind that while the task may seem to be simple, many hazards are sometimes not as apparent as one would imagine.
For the purposes of this assignment and hazard analysis, you will assume you have had a flat or blowout while driving alone, on a busy highway or road, and in inclement weather (i.e., rain, snow, fog). You may also choose whether this incident involved another vehicle.
Ensure you fully read all of the instructions. There are two documents to be prepared and submitted:
Part I: The Job Hazard Analysis (JHA) form, and
Part II: A document explaining the reasoning behind how you completed the JHA form.
For Part I, you must complete the following:
Step 1: List the specific manufacturer instructions for a vehicle of your choosing that you currently own (e.g., car, motorcycle, truck, ATV, RV, boat trailer) for the changing of a tire. The list does not have to be verbatim, but the critical sequence of events should be included.
Step 2: Refer to your list of specific instructions and as you review the section in Chapter 9 of your eTextbook titled A Hazard Analysis and Risk Assessment Guide as well as Appendix 2 of OSHA’s Job Hazard Analysis to determine each hazard and associated risk. Keep in mind you are looking to identify which conditions could be causal factors when changing a tire. You must identify at least five hazards.
Step 3: Use the Risk Assessment Matrix, including Probability and Severity Codes, Table 9.9 in Chapter 9 of the eTextbook to complete the risk assessment of each hazard in your list.
Step 4: Evaluate the hazard, associated risk, and risk assessment code (RAC) for each step/task in your list as you complete the Unit V Job Hazard Analysis Form . Ensure you have identified at least five hazards.
All sections of the form for each step/task must have an assigned RAC. As an example, if you determine that the occurrence probability for a step is Occasional and the severity of consequences is Critical, the RAC would then be S – Serious.
For Part II: In a separate Word document, you will explain the reasoning behind how you completed the Job Hazard Analysis Form. You must address at least the following:
Briefly summarize the conditions you have chosen for this scenario (i.e., location, type of inclement weather, single or multi-vehicle incident).
Discuss the concepts of risk and hazards and the types of risk as they apply to this scenario.
Explain how the components of risk management were used to identify the hazards and determine the risks.
Conclude Part II with a summary of how the applied process and resulting insight achieved in this activity can be universally applied in workplace safety management systems (SMS). What value does this process add to an SMS?
This project will be continued in the next unit. You are encouraged to review the next unit’s project instructions to ensure you are building a solid foundation for that assignment and that you have clearly established the conditions for this scenario.
Part II of this project must be at least one page. The title page (if used), or any reference pages do not count toward the page requirement. In addition to the Chapter 9 material and the OSHA Appendix listed above, you must use at least one peer-reviewed or academically reliable source to support your discussion. Steps
Create an ethical analysis related to your company’s specific crisis identified in Module 2 (word document attached). Such an analysis must of course consider the elements and anticipated actions embodied in the crisis plan, and should also consider the larger ethical context of the conditions (ethos, environment, etc) and normal operating principles driving the company, as these influence the likelihood, characteristics, and impact of any crisis. All ethical lapses represent mismanagement of one form or another, but not all mismanagement is an ethical lapse. Thus, please focus on “true” ethical lapses, and not just mismanagement per se. Do not just answer the questions above; they are for you to use as a guideline to assist in creating your analysis. . The analysis should be a short exposition, less than one page.
